Preparing for your appointment

Thank you for choosing Vitalize for your healing. We want to ensure your comfort and have outlined what you can expect—drinking plenty of water before and after your treatment is a good idea. Please prepare to dress down for your treatment or wear loose clothing.

If you would like to use insurance, we recommend contacting your insurance company at the number on the back of your card to inquire about the details of your plan. We have included a resource that will support gathering the relevant information.

Please upload your insurance and demographic information to the patient portal after booking your first clinic session. We will bill your insurance company directly using the information you upload. In the event that we receive a denial for services, we will make every attempt to appeal and receive payment.

Prior authorization and/ or medical necessity review may be required by your health insurance plan for your services at Vitalize. If your plan requires prior authorization for acupuncture, chiropractic care, massage therapy, or physical therapy services please notify us at or before your first visit.

Ultimately, if we are unable to obtain payment for your services, the responsibility for payment will be yours. We send invoices for balances due using the client portal to the email address you use to make your appointments online. Please be on the lookout for our emails.

We know that life can get in the way of best-laid plans. We ask that you notify us 24 hours in advance if you cannot keep your appointment time so that others can get the treatment they seek. Please be aware that there is a $60 fee for cancellations with less than 24 hours notice. If, however, we are able to book your canceled spot regardless of when you cancelled, you will not be charged a fee.

Our patient portal will also email you with medical history forms for you to complete after your first appointment is booked. You may fill them out via the client portal, and they will remain there with your health insurance and payment information for you and your provider to view.

Acupuncture

Dr. Auden Foxe L. Ac. and Julie Lee L. Ac. are in-network with most major insurance carriers including but not limited to Kaiser, CHP, Pacific Source, Aetna, MODA, Blue Cross Blue Shield of Oregon (and many other states), Providence, and United Healthcare (out-of-network with Medicare under United Healthcare).

We bill Motor Vehicle Accident + Workman's Comp claims. *Acupuncture requires a referral from an MD or DC in order to bill for Workman's Comp.

Please note that acupuncture services will remain out-of-network with First Choice, Multi Plan, Cigna, Health Net, and OHP. We are happy to bill as out-of-network for these companies

Vitalize + insurance

Our providers are In-Network with most major insurance carriers with a few exceptions which you will find below. If you have questions about your benefits or would like us to verify your insurance please call or email us with picture of the front and back of your insurance card, DOB and address. If you have any questions about insurance we urge you to take a look at our blog or call or email us. We know navigating benefits can confusing and we're here to help you untangle it.

Usually yes! Acupuncture is typically covered under most major insurance plans. There are some exceptions depending on the plan, insurance company or referral requirements. Please clarify coverage to help understand what your potential out of pocket costs may be required.

Insurance Payment + timelines 

We do our best to ensure your benefits are verified and this information is passed onto you before your appointment however, in some cases this may not be possible. We encourage you to communicate with us about any insurance related concerns, information or questions before your appointment.  Insurance companies often have 90 days or more to respond to a provider claim. Ultimately, the cost of the appointment may be your responsibility.  If you need urgent details, your insurance company may also be able to provide information regarding your plan coverage.  There are many important questions to ask your insurance provider to assist in confirming coverage. Let us know if you have questions.

Appointment Payment + timelines 

We ask that patients pay for their portion of the appointment (copay, co-insurance, deductable, etc.) in the office at time of service. Please be prepared to pay for your visit and any additional herbs or supplements that might be prescribed.  We accept cash, credit cards, and most HSA cards.

Our priority is always your care and peace of mind, if any issues or unexpected costs arise we will communicate this information to you as soon as is possible. We will always work with you to ensure you are supported in both care and financial responsibility. We encourage you to communicate any concerns or questions you might have with our staff in order to avoid unnecessary stress. 

Exceptions + specifics

All payments are due at time of service. It is each patients responsibility to know and understand your copay payments and deductibles. We will send an invoice prior to collections as noted in the signed agreement. We value and appreciate your diligence and understanding.
